Medicare Expands Service Areas for Telemedicine

The American Telemedicine Association is applauding a decision from the Centers for Medicare and Medicaid Services to expand rural coverage of telemedicine services, which CMS calls telehealth, reimbursable by Medicare.
CMS in a final rule setting the physician and clinical laboratory fee schedules for CY 2014 modifies its definition of Health Professional Shortage Areas to “allow for the appropriate inclusion of additional HPSAs as areas for telehealth originating sites,” according to the final rule, with comment period, available here and being published on Dec. 10.

Hospital groups unhappy about new Medicare flat rate for clinic visits

Hospital groups generally are not pleased with the new CMS rule converting the current five-level, intensity-based payment system for clinic visits into a single outpatient visit code. But at least one hospital official isn't too worried.
Ken Fisher, chief financial officer of University of Iowa Hospitals and Clinics, said the Iowa City-based 692-bed hospital, which averages more than 800,000 outpatient visits a year, did its own assessment and found it would fare the same or even a little better financially under the flat payment rate.

The federal HITECH program has served as an effective stimulus to get thousands of hospitals into digital recordkeeping. As of Oct. 1, CMS said that 71% of hospitals have attested to achieving Stage 1 meaningful use of EHRs, up by nearly 83% from a year ago.

Bane or boon? GOP, Dems spar over Medicare Advantage cuts at hearing

Congressional Republicans and Democrats on Wednesday offered starkly divergent views about the effects of cuts to the Medicare Advantage program that were part of the Patient Protection and Affordable Care Act.
During a hearing held by the House Energy and Commerce Committee's Health Subcommittee, Republicans portrayed the reductions to the program—which were designed to bring per-beneficiary costs in line with traditional Medicare—as evidence of harm caused by the federal healthcare overhaul. In particular, they seized on the narrowing of Advantage provider networks as proof that President Barack Obama misled the country when he promised that everyone would be able to continue seeing their doctor—even though these decisions to narrow the networks are being made by private insurers for financial reasons, not by the government.

Facebook dominates mobile apps, survey says

In a report released Wednesday, research firm Consumer Intelligence Research Partners asked mobile phone users for the names of the three apps they use most frequently. Facebook scored the top spot, ranked by 45 percent of those polled. Twitter came in second at 13 percent, followed by Candy Crush at 11 percent.
"Facebook just dominates mobile phones, in terms of most frequent use, not just downloads," CIRP partner and co-founder Mike Levin said in a statement. "For most other apps, including some well-known ones like YouTube and Pandora, fewer than 10 percent of phone buyers included them among the most frequently used."

Online spending on Black Friday increased 15 percent to a record $1.20 billion as more consumers opted to shop from their couches rather than battling long lines at stores. Enlarge image Online Black Friday Shopping Total e-commerce sales reached $20.6 billion in the first 29 days of this holiday season, according to ComScore Inc. That’s about a 3.1 percent increase from Nov. 1 to Black Friday last year, the research firm’s data showed, noting that 2013 numbers include a few more shopping days because Thanksgiving fell on a later date this year.

As out-of-pocket costs increase for consumers, there’s a big push for hospitals to improve price transparency. Although the demand represents a trend in healthcare, particularly among startups, it could be part of the future of healthcare once providers and payers and groups can untangle the complexities. A study published in the Journal of American Medical Association took on the issue by calling 20 hospitals in Philadelphia. It wanted to compare the ease of getting the price of parking over the phone compared with getting the cost of a medical procedure — in this case electrocardiograms.

Reform Update: Delay Medicaid DSH cuts, hospitals ask lawmakers

Hundreds of hospitals around the country have signed a letter to congressional leaders (PDF) asking them to delay cuts to Medicaid supplemental funding for the care of the uninsured and low-income patients.
More than $18 billion in cuts for the Medicaid disproportionate share hospital funding are expected to take effect between 2014 and 2019. Those cuts were mandated by the Patient Protection and Affordable Care Act, which was passed with the expectation hospitals would deliver less uncompensated care because all states would expand Medicaid eligibility to adults earning up to 138% of the federal poverty level. But the U.S. Supreme Court last year made Medicaid expansion optional for the states, and about half the states have not expanded their programs.

Chicagoland’s HIE Gets Ready to Roll

Four years after legislation paved the way for a massive health information exchange serving up to 89 hospitals in the greater Chicago region, the first wave of facilities will start hooking up in January.
Thirty-four hospitals are preparing to go live on the MetroChicago HIE with the first ones on board during the first quarter of 2014, says Daniel Yunker, CEO of Land of Lincoln Mutual Health Insurance Company and senior vice president of the Metropolitan Chicago Healthcare Council, which leads the HIE effort. Initial services, live by the end of the quarter, will include admissions/discharge/transfer document exchange, Direct Project secure messaging, a master patient index and record locator. These initial services are achievable for the first 34 hospitals and are sufficient to provide value in the first phase of the HIE, Yunker adds.

It's not all about location for health information exchange networks--physicians are influenced by other physicians with whom they interact with and have common patients with more than geographical location in HIEs, according to new research published in the Journal of the American Medical Informatics Association.
For the study, network effects among clusters of physicians based on their professional and geographical proximities were examined. A diffusion model was used to capture both indirect and direct network effects among groups, studied over a three-year period, for an HIE in Western New York by researchers from the State University of New York at Buffalo.
