GOP fears about IRS access to medical records disputed

Listening to recent statements from some congressional Republicans, you might think that the 2010 health law allows the Internal Revenue Service to have access to your medical records.
Not so, says the Department of Health and Human Services. “The Affordable Care Act maintains strict privacy controls to safeguard personal information. The IRS will not have access to personal health information,” said agency spokeswoman Erin Shields Britt.

Senators seek overhaul of hospital payment system

When it comes to Medicare hospital payment systems, the exceptions are the rule. Congress' nonpartisan investigative arm reported Thursday that 91% of hospitals paid by Medicare receive some dispensation or add-on to the program's standard payment system.
The Inpatient Prospective Payment System was designed to maximize “cost-control, efficiency and access” when it was launched 30 years ago, according to the Government Accountability Office report. But Congress has piled an accumulating number of exemptions and carve-outs for various types of hospitals.

Employers are increasingly recognizing they may be able to avoid certain penalties under the federal health law by offering very limited plans that can lack key benefits such as hospital coverage.
Benefits advisers and insurance brokers—bucking a commonly held expectation that the law would broadly enrich benefits—are pitching these low-benefit plans around the country. They cover minimal requirements such as preventive services, but often little more. Some of the plans wouldn't cover surgery, X-rays or prenatal care at all. For the third time since early 2011, the House of Representatives approved legislation to fully repeal the Patient Protection and Affordable Care Act. In a vote of 229 to 195, the lower chamber approved the bill from Rep. Michele Bachmann (R-Minn.), which has no chance of passing in the Senate. "Today is a sham. It's a waste of time," Rep. Rosa DeLauro (D-Conn.) told Modern Healthcare moments after the vote. "If we're to do something, we ought to be doing jobs. But this is a Circus here. This is Groundhog Day what happened here today."

To improve patient safety, hospitals should randomly test physicians for drug and alcohol use in much the same way other major industries in the United States do to protect their customers. The recommendation comes from two Johns Hopkins physicians and patient safety experts in a commentary published online in The Journal of the American Medical Association.
In addition, the experts say, medical institutions should take a cue from other high-risk industries, like airlines, railways and nuclear power plants, and mandate that doctors be tested for drug or alcohol impairment immediately following an unexpected patient death or other significant event.
