Government health care fraud is big business. According to the federal website, PaymentAccuracy, the government distributed nearly $65 billion in improper health care-related payments, and government agencies have been hard at work trying to put a stop to it — but with limited results.
Even though $1.7 billion was spent to combat fraud, waste and abuse in 2010, less than 10 percent of fraudulent dollars were recovered. This shouldn’t be surprising. Once the money is out the door and in the hands of criminals, it’s almost impossible to get back.

In four studies and a commentary published in JAMA Internal Medicine, the authors look at various aspects of doctors' dialogue with patients about prognoses, options and treatment preferences and find little consistency.

Content on health websites often too complex for patients

If you feel like you're lost in a sea of jargon when navigating healthcare-related websites, you're not alone. A new research letter published in JAMA Internal Medicine assesses readability of patient education resources found online, determining that such materials often are too complex for their intended audience
For the research, resources from 16 medical specialties were examined with software called Readability Studio, where readability was assessed using different formulas and indexes. All assessments showed that materials were too complex for the sixth-grade reading level; the average American adult reads at approximately a seventh- to eighth-grade level, according to the authors.

Listening to recent statements from some congressional Republicans, you might think that the 2010 health law allows the Internal Revenue Service to have access to your medical records.

Senators seek overhaul of hospital payment system

When it comes to Medicare hospital payment systems, the exceptions are the rule. Congress' nonpartisan investigative arm reported Thursday that 91% of hospitals paid by Medicare receive some dispensation or add-on to the program's standard payment system.
The Inpatient Prospective Payment System was designed to maximize “cost-control, efficiency and access” when it was launched 30 years ago, according to the Government Accountability Office report. But Congress has piled an accumulating number of exemptions and carve-outs for various types of hospitals.

Employers are increasingly recognizing they may be able to avoid certain penalties under the federal health law by offering very limited plans that can lack key benefits such as hospital coverage.
Benefits advisers and insurance brokers—bucking a commonly held expectation that the law would broadly enrich benefits—are pitching these low-benefit plans around the country. They cover minimal requirements such as preventive services, but often little more. Some of the plans wouldn't cover surgery, X-rays or prenatal care at all. Again, House votes to repeal reform law

For the third time since early 2011, the House of Representatives approved legislation to fully repeal the Patient Protection and Affordable Care Act. In a vote of 229 to 195, the lower chamber approved the bill from Rep. Michele Bachmann (R-Minn.), which has no chance of passing in the Senate. "Today is a sham. It's a waste of time," Rep. Rosa DeLauro (D-Conn.) told Modern Healthcare moments after the vote. "If we're to do something, we ought to be doing jobs. But this is a Circus here. This is Groundhog Day what happened here today."
